Working within a specialist Older Adult Community Mental Health Team, you'll be managing a caseload of clients over the age of 65 who have complex mental health problems both psychotic and non-psychotic in nature.

The post holder will have responsibility for the day-to-day delivery of the service to individual clients and their families and carers via a range of evidence-based interventions.

Those clients with a psychotic illness may be managed through Shared Care model or under our Enhanced Psychosis Pathway. This allows the sharing of responsibility, skills and knowledge which offers the opportunity for staff to develop their role whilst being supported by colleagues.

You will be working as part of a multi-disciplinary team alongside our Dementia pathway colleagues including shared MDT meetings and duty function. You'll be communicating with a wide variety of clients; professionals; partners and agencies.

The nursing role also includes medication management, administering of prescribed depot injections, new patient core assessments, risk assessments and participating in a duty rota covering both Dementia and Older Adult Service. The team is looking at specialist roles to ensure that we utilise our staffs skills and interests to the fullest.

The post holder will ensure that all organisational systems and processes are adhered to. The post holder will manage and organise competing and conflicting priorities and manage change in complex situations.

Supporting non-nursing staff with relevant clinical issues.

Line managing and professionally supporting peers and junior staff

Provide the nursing voice and participate as part of the multi-disciplinary team implementing, evaluating and reviewing all aspects of the service users care.

Advise, inform and support on the physical health and wellbeing of our clients including advocating and facilitating their access to other services when required ensuring that this is done in a way which respects their values and supports their rights.

Evaluate the effectiveness of interventions in relation to supporting the recovery and wellbeing of service users.

The successful applicant will manage a caseload of service users and deliver intervention's to meet each individual's needs.

The team share one large open plan office with other services, therefore have regular team events and developments to support a positive working atmosphere and collaborative working.
