Working within a specialist Older Adult Community Mental Health Team, you’ll be managing a caseload of clients over the age of 65 who have complex mental health problems both psychotic and non-psychotic in nature. The post holder will have responsibility for the day-to-day delivery of the service to individual clients and their families and carers via a range of evidence‑based interventions. Those clients with a psychotic illness may be managed through Shared Care model or under our Enhanced Psychosis Pathway. This allows the sharing of responsibility, skills and knowledge which offers the opportunity for staff to develop their role whilst being supported by colleagues. You will be working as part of a multi‑disciplinary team alongside our Dementia pathway colleagues including shared MDT meetings and duty function. You’ll be communicating with a wide variety of clients, professionals, partners and agencies. The nursing role also includes medication management, administering of prescribed depot injections, new patient core assessments, risk assessments and participating in a duty rota covering both Dementia and Older Adult Service. The team is looking at specialist roles to ensure that we utilise our staff’s skills and interests to the fullest. Responsibilities Manage a caseload of older adults with complex mental health problems, delivering evidence‑based interventions. Use the Shared Care model or Enhanced Psychosis Pathway to manage psychotic illnesses. Work collaboratively within a multi‑disciplinary team, including MDT meetings and duty function. Carry out medication management, depot injections, core assessments, risk assessments and participate in the duty rota. Ensure adherence to organisational systems and processes, managing competing and conflicting priorities. Support non‑nursing staff with relevant clinical issues, providing guidance and collegial support. Line‑manage and professionally support peers and junior staff. Represent the nursing voice in multidisciplinary team meetings, evaluating and reviewing all aspects of service user care. Advise, inform and support clients on physical health and wellbeing, advocating access to other services in a manner that respects their values and supports their rights. Evaluate the effectiveness of interventions in relation to service user recovery and wellbeing. Deliver tailored interventions to meet each individual’s needs. Qualifications & Experience We are looking for enthusiastic individuals who can work confidently on their own as well as supporting the team. Good communication and problem‑solving skills are essential, as well as knowledge of severe mental illness and the challenges this brings in daily life. You also need to be a driver with access to a car that can be used for business purposes.
