We are seeking an experienced and motivated individual to join a supportive forward-thinking urology cancer nursing team at Ashford & St peter's Hospitals

The post holder will lead on the delivery of high quality, patient centred care for men living with and beyond cancer.

The post focuses on providing expert clinical, psychological and educational support to patients on the prostate cancer PSFU surveillance pathway, ensuring safe, efficient and holistic management through out their cancer journey.

You post holder will work autonomously and collaboratively with the wider MDT, supporting patients who have completed active treatment and are transitioning to a self management pathway.

There are flexible working options with a strong focus on staff wellbeing.

Act as the key clinical contact for men on the PSFU pathway.

Monitor patients remotely through PSA surveillance, identifying and acting promptly on abnormal results or patient concerns.

Deliver nurse-led clinics to include HNA , treatment summary and discharge clinics.

Provide expert advice to patients, families and healthcare professionals regarding treatment side effects, symptom management and holistic needs.

Contribute to evaluation and ongoing service development of the PSFU surveillance pathway in accordance with national standards.

Work in partnership with CNS colleagues, cancer support workers, urologists & oncologists to ensure seamless care transitions

To be a resource both within the trust and the community for clinical advice and information.

With the Lead Nurse contribute to clinical governance within the trust for cancer patients.

Support the Prostate Cancer PFSU pathway, ensuring a safe transition to the surveillance program with effective follow up

Deliver nurse led clinics

Promote high standards of specialist care for men with a diagnosis of prostate cancer, ensuring a quality experience.

Deliver education programmes supporting the Living with Beyond Cancer NHSE framework.

Work autonomously within the MDT to support decision making.

Support consultants with the delivery of a new diagnosis

Discuss investigations and treatment for cancer with sensitivity, knowledge and expertise.

Deliver clinical care and management in line with local and national guidance and best practice

Contribute to service development with audit and quality improvement initiatives

This is an opportunity for a nurse with relevant experience in prostate cancer nursing to further develop their skills and knowledge, training and support will be provided. There will be opportunities for ongoing educational development..
