Occupational Therapist

Delivery of safe and effective rehabilitative patient care in patients homes working in partnership with colleagues in the multi disciplinary team supporting the efficient use of resources across the whole system delegating duties as appropriate. Work flexibly across localities and occasional weekends to ensure the requirements of the service contract are met. Work independently as part of an interdisciplinary team to prioritise, assess, plan and implement patient specific OT programmes of care/rehabilitation,analysing patient needs and making judgements about best practice contributing to the continuous assessment of patients. Undertake assessment of patients as an autonomous practitioner using clinical reasoning skills, knowledge of evidence based practice and professional expertise.
