Job summary

The Bordon and Petersfield Community Older Persons Mental Health (OPMH) Team operates from the main team base at Chase Hospital and covers Petersfield, Bordon and some of Waterlooville working 9-5 Monday - Friday. The team is approachable and open. We support each other and have developed a culture of honesty and integrity.

This is a lively, dynamic and busy multi-disciplinary team which provides care coordination and recovery focused care for individuals with both organic/functional mental health needs. This is an opportunity to demonstrate your skills as a Healthcare Support Worker, who is focused on continually improving outcomes for patients.

You will play an important role in delivering excellent care for our patients and working as part of the team to provide the right support and assistance to our patients in their recovery. You would be joining a supportive team that work together closely.

Due to the nature of this post, you will need to carry out home visits and attend patient meetings therefore, a car driver is essential with access to own car.

Main duties, tasks & skills required

The Community Mental Health Support Worker will work as a core member of the Memory Assessment Services as well as supporting functional patients within the community. The primary aim is to support those patients who have been given a specialist assessment including dementia or a functional illness offering post post-diagnostic support. This will include treatment initiation, titration of medication post diagnostic care and support, care planning, risk management and signposting to other professional and voluntary services.

The support worker will work in accordance with an agreed plan of care and under the direction and supervision of professionally qualified staff to promote the maintenance and achievement of recovery focussed goals for the service user including post diagnostic support.

Qualifications
Essential
  • Literacy and numeracy skills
  • Full driving licence and use of a vehicle
  • Experience of working with older persons both organic & functional preferably in a community setting
  • Understanding of organic and functional illness.
Desirable
  • Completion of NVQ Level 3 in Health and Social Care
  • GCSE in English & Mathematics at level C or above (or equivalent)
  • Care Certificate
