Job Summary
We're a dedicated supported living and outreach service, focused on empowering individuals with mental health needs and learning disabilities to live independent, fulfilling lives. Our team works 24/7 to provide consistent, person-centred support.
As a Disability Support Worker, you will play a crucial role in enhancing the quality of life for individuals with disabilities. Your primary responsibility will be to provide compassionate care and support, enabling supported individuals to achieve their personal goals and maintain independence in their daily activities. You will work closely with supported individuals, their families, and other professionals to implement tailored care plans that address individual needs.
Responsibilities
- Deliver best practices in care and support meeting quality and regulatory standards.
- Provide person-centred care that promotes independence, choice and dignity.
- Assist supported individuals with daily living activities, including personal care, meal preparation, and household tasks.
- Develop and maintain positive relationships with supported individuals to foster trust and communication.
- Collaborate with management and the team to create and update individual care plans and risk assessments based on supported individuals' needs and preferences.
- Provide emotional support and encouragement to supported individuals, promoting their self-esteem and independence.
- Monitor and document supported individuals' progress, reporting any changes or concerns to relevant professionals.
- Utilise IT systems for record-keeping and communication purposes effectively.
- Participate in training sessions and professional development opportunities to enhance your skills and knowledge in disability support.
- Demonstrate positive teamwork qualities by mentoring new staff members and contributing to a positive team environment.
Requirements
- Strong communication skills, both verbal and written, to interact effectively with supported individuals, families, and colleagues.
- Experience or knowledge in developing care plans and risk assessments tailored to individual supported individuals' needs is highly desirable.
- Experience of supporting individuals with additional needs, especially autism and mental health.
- Proficiency in IT systems for documentation and reporting purposes.
- Previous experience in a similar role or within the health and social care sector is essential. Minimum 24 months experience in the UK
- A compassionate nature with a genuine desire to make a difference in the lives of individuals with disabilities.

- Knowledge of criminal and sexual exploitation.
- Excellent communication, flexibility and resilience with the ability to work a range of shifts
