Our SMI-LE service provides tailored support to people in the community who are experiencing mental health challenges. This includes assistance in key areas such as (mental health and emotional well-being; loneliness and social isolation; social housing; welfare, benefits, and debt management and employment).

Support is personalised to each individual and can be provided in hospitals, at home, within the community, or through remote services. You'll also signpost and refer service members to other local services for continued support. Travel across Bradford and Craven is required, and you'll work closely with the wider team to ensure coordinated, person-centred care.

- Provide support that meets the service member needs. This support will be around client well-being, such as employment, welfare and benefits, social housing, money management, social isolation, and bereavement.
