Job Summary

We are seeking a compassionate and dedicated support worker to provide high-quality support to patients within care homes and home care settings. The successful candidate will play a vital role in assisting with daily activities, supporting care plans, and ensuring the wellbeing of individuals under our care. Duties

  • Assist patients with personal care tasks such as bathing, dressing, and mobility support
  • Support the implementation of personalised care plans in collaboration with healthcare professionals
  • Provide companionship and emotional support to patients, promoting their mental wellbeing.
  • Help with medication administration as directed by healthcare staff
  • Maintain accurate records of care provided using IT systems and documentation tools
  • Support with light domestic duties to ensure a clean and safe environment
  • Drive between care settings or patient homes as required, ensuring timely and reliable transportation
  • Communicate effectively with patients, families, and healthcare teams to ensure coordinated care

Skills

  • Good communication skills in English, both verbal and written
  • Strong organisational skills to manage multiple responsibilities efficiently
  • Excellent interpersonal skills with a caring and empathetic approach
  • Experience working within a care home or home care environment is advantageous
  • Knowledge of care plans and procedures related to patient support is beneficial
