We are seeking a motivated and compassionate Band 6 Community Care Coordinator to join our Community Mental Health Team in Keighley. This is an exciting opportunity to make a real difference in the lives of service users by delivering high-quality, person-centred mental health care.As a Care Coordinator, you will manage a defined caseload, working autonomously and collaboratively within a multidisciplinary team.
