We are looking for someone with the right values and behaviours who can help ensure our workplace values, developed by staff and individuals who use our services, are at the heart of everything we do.
The role will be working with our Reablement and Rapid Response Teams, accepting / processing referrals and developing call lists for the staff who already have an established rota's. You'll liaise with social workers & hospital discharge planners daily.

Shifts are scheduled between8:30am and 8:30pm, with some weekend working required on a rota basis.
An Enhanced DBS Check is required.
