This is a unique opportunity to be part of a pioneering service that's transforming how we deliver acute care bringing hospital-level treatment into the comfort and safety of patients' own homes.

What's the Virtual Ward all about?

Our Virtual Ward supports people living with frailty or acute illness to stay safely at home or return home sooner after a hospital stay.

Educate and empower hospital staff to identify suitable patients who meet the criteria for Virtual Ward care.

Supporting the Virtual Ward Matrons with the implementation of a Virtual Ward case management system.

Develop a comprehensive and cohesive plan for supporting discharge flow which meets the strategic direction of the service/team and minimises unnecessary disruption to stakeholders involved in the process and is operationally sound.
