Your role and responsibilities 

  • Oversee referrals from community and hospital teams, ensuring they're allocated to Reablement Support Workers through the Home First scheduling system.
  • Act as the triage point in Liquid Logic, flagging cases that need attention and keeping workflows moving.
  • Work closely with Team Leaders to resolve allocation challenges, using data and insights to find practical solutions.
  • Apply up-to-date knowledge of legislation, policy, and procedures so workflows meet statutory and performance requirements.
  • Help adapt systems to respond to urgent or priority work and support change processes when needed.
  • Take part in supervision, team meetings, training, and development opportunities.
  • Maintain accurate records in Liquid Logic to ensure everything is documented properly.
  • Report allocation difficulties to the Registered Manager and assist in finding solutions.
  • Support complaints resolution by collecting and collating relevant information.
  • Champion excellent customer care by keeping customers informed about timescales and next steps.
  • Manage Home First referrals in line with access criteria and Provider of Last Resort responsibilities.
  • Promote integration with Health colleagues and build strong partnerships across services.
  • Monitor Home First activity and ensure smooth operations.
  • Foster strong relationships between Home First teams and referring organisations.
  • Make sure customers are allocated quickly to reduce wait times for assessment, reablement, or domiciliary care.
  • Oversee the scheduling system to reduce workflow blockages and maximise capacity.

The team 

Home First provides short-term reablement care and support to people living in their own homes. Our service helps individuals regain independence following a hospital stay, illness, or crisis at home, and also works to prevent unnecessary hospital admissions.

You'll collaborate with colleagues across Adult Community Services (ACS) Cluster Teams, Independent Providers, Health partners, Borough and District Councils, Multi-Agency Safeguarding, and a wide range of voluntary sector organisations.

This is a hybrid role, with midweek office days dedicated to team collaboration, sharing best practice, and supporting each other. Every other weekend, you'll work from home, covering the Home First county alongside colleagues from other area teams.
