Treating Tobacco Dependency Advisor - Inpatients - NBT/ UHBW Applicants Only
We are looking for a motivated individual to join our Inpatient team as a Treating Tobacco Dependency Advisor at Weston General Hospital (WGH).

This is a permanent full, or part-time role, which would also be very suitable as a developmental secondment opportunity for applicants currently in band 3 patient-facing roles.

The postholder will have excellent listening and communication skills and be able to quickly build trusting and collaborative relationships with patients, to help inpatients to stop smoking or to reduce the harm from tobacco during an inpatient admission.

This role is based at Weston General Hospital. The team operates across departments within North Bristol NHS Trust (NBT) and UHBW). The service core hours are Monday-Friday.

Main duties of the job

The Advisor will deliver specialist motivational and behavioural support, develop personalised care plans in partnership with patients and facilitate access to appropriate pharmacotherapy and e-cigarettes/vouchers. The role will contribute to tackling health inequalities and focuses on engaging with inpatients, offering them support to motivate to change their behaviour and to achieve their stop smoking goals.

The Advisor will build and maintain effective working relationships with all professionals within the multidisciplinary team and act as a patient advocate to ensure that tobacco dependency treatment needs are met.

The Advisor will support the development of the day to day running of the service to ensure that patients receive safe, evidence-based care. The Advisor will manage a delegated caseload of patients, maintain accurate patient and service data records.

Care/Service Provision

  • To plan and deliver stop smoking advice and support sessions, including advice on stop smoking medications, working within recommended protocols and guidelines. This will involve working with individuals in-person and on the phone and group work sessions in a variety of settings and venues.
  • Identify and approach service users who are eligible for tobacco dependence conversations and treatment whilst an inpatient.
  • Provide specialist behavioural and motivational support as part of personalised care plans.
  • Refer patients to community services to continue with the stop smoking attempt.
  • Develop individually tailored support plans including recommending treatments based on assessed needs.
  • Provide general health, lifestyle advice and support to people using the service.
  • Participate in educating inpatients about the benefits of stopping smoking whilst in hospital and upon discharge.
  • Deliver Carbon Monoxide (CO) monitoring, record the results and ensure the patient understands the result.
  • Engage with the targeted treating tobacco dependency intervention, keeping accurate records and enable good data collection. Always maintain accurate up to date and comprehensive records.
  • Ensure that the relevant equipment is provided and maintained on an agreed schedule. Report any failings of equipment or other environmental factors to the Clinical Lead.
  • Respond effectively to referrals within stated timescales.
  • To be involved in the monitoring and evaluation of patient/client feedback, offering recommendations as part of the team to improve services.
  • Be aware of how to report incidents, concerns or complaints and escalate any concerns or complexities to the Clinical Lead.

Monitoring And Administrative Tasks

  • To contribute to the monitoring and evaluation of the service, including the completion of the minimum data set requirement. This will include following up patients who do not attend for planned support.
  • Maintain accurate patient records in line with Trust guidelines.
  • Ensure administrative duties are carried out efficiently and effectively in order to meet the needs of the service.

Team Working

  • Contribute actively to open discussions of how local services can be improved, supporting improvements within the local community, and demonstrating an understanding of the barrier's individuals are experiencing.
  • Play an active role in the development of the Treating Tobacco Dependency service targets and work collaboratively with team members to share best practice and information to achieve these.
  • Work closely with the local stop smoking services and the local Public Health teams to ensure the work links to wider strategy.
  • Meet regularly with the Clinical Lead to assess and direct progress.
  • To maintain good working relationships with all levels of staff.
  • To liaise with others in the health and social care community in order to promote the tackling tobacco dependence programme and treating tobacco dependency intervention and encourage appropriate referrals.
  • To support the delivery of tobacco dependency training to the multidisciplinary team.

Managing Self

  • Undertake relevant training to maintain the skills and knowledge as necessary through participation in continuous development.
  • Work with indirect minimal supervision from the Clinical Lead, operating at all times within the assessed level of competence. Clear professional boundaries and limitations will be discussed and monitored by the Clinical Lead.
  • Work within the agreed and relevant policies and procedures and know how to access them.
  • Seek advice and support as and when appropriate.
  • Plan own activities, sessions and care plans to meet the needs of patients within the service.

Qualifications
Essential criteria

  • GCSE level English and Mathematics or equivalent.
  • NVQ level 3/4 or equivalent health related studies OR substantial successful experience delivering stop smoking interventions/support.

Desirable criteria

  • Motivational Interviewing/supporting behaviour change qualification

Experience
Essential criteria

  • Recent experience of working with patients in the NHS in an Acute, Community or relevant setting, within the last 2 years
  • Ability to work independently and as part of a team.
  • Experience of communicating health information and advice using a person-centred approach

Desirable criteria

  • Experience of providing healthcare, healthcare advice, health promotion advice on a one to one advice interventions basis or as part of a group setting

Skills
Essential criteria

  • A good level of understanding of evidence-based practice in relation to treating tobacco dependency, the consequences of smoking and benefits to quitting
  • An understanding of human behaviour in relation to making healthy lifestyle changes and decisions related to tobacco dependency
  • The ability to build and maintain effective working relationships with professionals/individuals from a variety of different professional background
  • Be able to provide evidence of excellent listening and communication skills (both written and verbal).
  • The ability to quickly build rapport with patients with varying health needs and from a variety of socio-economic backgrounds
  • Good standard of computer skills
  • Ability to act, where appropriate, as the patient advocate to ensure that tobacco dependency treatment needs are met during an inpatients stay and on discharge to the community

  • Good organisational skills and the ability to prioritise workload and coordinate own schedule without direct supervision.
