Discharge to Assess Coordinator

In this specific Coordinator role, as part of the Hospital Discharge Team, you will work with service users, families and carers who are currently in hospital. You will be supporting their appropriate and safe discharge, working for and with individuals, taking care to understand their unique circumstances and priorities.
Championing the principles of choice and control, you will also support people to make their own decisions; and to be empowered to have services organised, and risks managed, in ways that are right for them. You will be supporting people and their families / carers who are being discharged from hospital, making a direct difference to their lives by ensuring they receive the relevant aftercare and support.
You will be highly motivated, with lots of initiative, and will demonstrate the ability to adapt your interventions in response to all kinds of people, including external and internal partners. You will be naturally collaborative, bringing to the role a flair for building relationships, and working in partnership.
You'll be responsible for maintaining and updating information systems and databases to ensure that service transactions are completed accurately. You will provide advice to customers, contacts, external organisations and colleagues ensuring that they receive appropriate guidance about policies, procedures and processes whilst ensuring effective working relationships are maintained.
You must be passionate about person centred Social Care, you will also have a proven talent for involving people in decision making and building on their strengths. In return, we will build on your strengths, with excellent opportunities to develop and progress.
