Role Description
This is an on-site, full-time role for a Dementia Care Manager (DCM) located in Bath. Our beautiful luxury yet homely and kind home needs a DCM who has demonstrated not only the relevant knowledge but also skills of implementing innovative ways to provide an individualised care to our residents.
The DCM will be responsible for managing the community and the team, keeping up to date with Hallmark standards and guidance, providing training to our team, delivering elder care services, and coordinating with social services and medical professionals. The DCM will ensure that our residents in our dementia community receive personalized, high-quality care and support.
Qualifications and Requirements
- Case Management and Auditing and monitoring
- Experience in Elder Care, MCA and DoLs, and Senses plan
- Training skills
- Excellent communication and interpersonal skills
- Ability to lead and work collaboratively within a team
- Relevant experiences and high level of understanding about Dementia are needed.
- Implementing ways and innovating procedures to support residents and our team
- Relevant certifications and qualifications in elder care or social services
There will be a second to noe induction to equip the successful candidate.
