We are seeking to appoint a passionate, person centered and highly motivated clinician to be part of the physiotherapy service in the Frailty pathway from ARI into the community.
The successful applicant will be joining a well-established, successful and friendly team, working alongside the other Band 7 Physiotherapists in Frailty. We are looking for an individual who is highly motivated and flexible in their approach, and is able to demonstrate excellent clinical leadership in the assessment and management of patients with Frailty.
Using their clinical expertise the successful candidate will work collaboratively to provide safe and effective care. They will work with a diverse range of patients with Frailty, both those in the Frailty pathway and those elsewhere in hospital. They will help facilitate early discharges, working as a core member of the MDT. They will develop and maintain strong relationships within the Physiotherapy team, the MDT on the wards and other professional groups and services across NHS Grampian. They will play an integral role in assisting teams and colleagues to confidently apply the home first principles.
This post holder will be part of a newly developing service which will be providing frailty services from 7am 8pm, 7 days a week. They will need to work collaboratively to ensure the delivery and embedding of change. They must be able to work well under pressure and be able to challenge the multi-disciplinary team, in a constructive manner to ensure safe and appropriate patient care is offered at all times. They will be responsible for the line management and performance management of a team of staff.
They will be a core part of our weekend frailty service at commencement of this post as NHS Grampian's physiotherapy team is proactively working towards services over 7 days.
