Hours:
Monday to Friday, 8:30 AM 4:30 PM or 9:30 AM 5:30 PM (On-call duties as required hrs per week.

Job Description Summary (Main Duties And Responsibilities)
As a
Specialist Clinical Pharmacist
, the post holder will play a pivotal role within the pharmacy team, contributing to the delivery of a
safe, effective, and patient-centred pharmacy service
across the Trust.

Key Responsibilities Include

  • Clinical Ward-Based Services:
  • Delivering a high-quality, patient-focused clinical pharmacy service on wards, ensuring the safe and effective use of medicines.
  • Supporting Timely and Safe Discharges:
  • Facilitating early discharge and reducing hospital length of stay by:

  • Optimising patients' drug therapy.

  • Providing tailored education on medicine use, including support with self-medication and assessing the need for adherence aids.
  • Offering comprehensive counselling to patients about their medications prior to leave and discharge.
  • Ensuring continuity of care through robust discharge planning and communication with community healthcare providers.

  • General Pharmacy Duties:

  • Participating in wider clinical pharmacy and departmental responsibilities as required, supporting the overall objectives of the Trust's pharmacy service.
