Responsibilities

  • Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
  • Monitor patients' health by observing vital signs and reporting any changes to healthcare professionals
  • Administer medication following prescribed care plans while maintaining compliance with HIPAA regulations for confidentiality
  • Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking
  • Support patients with mobility using equipment such as Hoyer lifts or resident's personal assistive devices
  • Conduct patient observation to ensure safety and comfort, noting any concerns or unusual behaviors
  • Perform light cleaning tasks to maintain a safe and hygienic environment in clients' homes or group settings
  • Assist with transportation to appointments or community activities, promoting social engagement and independence
  • Follow established care plans meticulously, documenting care provided and patient responses accurately
  • Support clients with specialized needs such as dementia, Alzheimer's, autism, or developmental disabilities through tailored approaches
  • Engage in ongoing training on CPR, first aid, hospice care, and other relevant certifications to ensure high-quality support
  • Collaborate effectively with healthcare teams, social workers, family members, and other caregivers to coordinate comprehensive care

Experience

  • Prior experience in home health care or assisted living environments is highly preferred
  • Demonstrated knowledge of patient monitoring techniques and vital signs assessment
  • Experience working with individuals with developmental disabilities, dementia, Alzheimer's disease or memory care needs is advantageous
  • Familiarity with nursing home procedures, long-term care practices, and hospice or palliative care settings is beneficial
  • Proven ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary
  • Strong understanding of HIPAA regulations to ensure patient confidentiality at all times
  • Experience in meal preparation and medication administration within a caregiving context is desirable
  • Ability to work effectively in group home settings or community-based environments with diverse populations
