Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patients' health by observing vital signs and reporting any changes to healthcare professionals
- Administer medication following prescribed care plans while maintaining compliance with HIPAA regulations for confidentiality
- Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking
- Support patients with mobility using equipment such as Hoyer lifts or resident's personal assistive devices
- Conduct patient observation to ensure safety and comfort, noting any concerns or unusual behaviors
- Perform light cleaning tasks to maintain a safe and hygienic environment in clients' homes or group settings
- Assist with transportation to appointments or community activities, promoting social engagement and independence
- Follow established care plans meticulously, documenting care provided and patient responses accurately
- Support clients with specialized needs such as dementia, Alzheimer's, autism, or developmental disabilities through tailored approaches
- Engage in ongoing training on CPR, first aid, hospice care, and other relevant certifications to ensure high-quality support
- Collaborate effectively with healthcare teams, social workers, family members, and other caregivers to coordinate comprehensive care
Experience
- Prior experience in home health care or assisted living environments is highly preferred
- Demonstrated knowledge of patient monitoring techniques and vital signs assessment
- Experience working with individuals with developmental disabilities, dementia, Alzheimer's disease or memory care needs is advantageous
- Familiarity with nursing home procedures, long-term care practices, and hospice or palliative care settings is beneficial
- Proven ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary
- Strong understanding of HIPAA regulations to ensure patient confidentiality at all times
- Experience in meal preparation and medication administration within a caregiving context is desirable
- Ability to work effectively in group home settings or community-based environments with diverse populations
