Job Title Lead Practitioner, Specialist Older Adults' Mental Health Service Employer Sussex Partnership NHS Foundation Trust Location Brighton General Hospital, Brighton & Hove Job Type Full‑time Seniority level Mid‑Senior level Job Function Health Care Provider; Mental Health Care Responsibilities Carry out initial assessments and manage a caseload of older adults with dementia and other mental health conditions. Provide professional input to the multidisciplinary team. Maintain high standards of documentation. Participate in professional supervision and regular CPD sessions. Develop skills in assessment, planning and service user management. Forge links with the third sector and specialist teams within the Trust. Qualifications & Experience Background in Nursing, Occupational Therapy or Social Work. Experience working with older people with dementia and other mental health conditions. Empathy, motivation, enthusiasm, creativity and resourcefulness. Strong communication skills and ability to develop working relationships with carers. Benefits £2,000 Golden Hello relocation incentive for Band 6 nurses & practitioners. Flexible working arrangements and supportive workplace culture. Working Hours Monday to Friday, 9 am to 5 pm.
