Weare looking for an existing General Practitioner to work with the Community andOlder Peoples Medicine Services, two sessions a week, to support a clinicalcaseload of patients undergoing rehabilitation alongside a multidisciplinaryteam to promote recovery of function and a return to independent living wherepossible.Main duties of the jobThe postholder will work as an integral member of the intermediate care team as the named clinician for a cohort of 10-15 patients. They will be expected to oversee and coordinate a Comprehensive Geriatric Assessment for patients who have either stepped up from the community to avoid acute hospital admission or are clinically stable following and acute admission.It is expected that the post holder will provide clinical sessions within the intermediate care unit/rehabilitation ward for 2 sessions per week. There may be occasionalstudents working in the clinical areas.
