Key Responsibilities Long term condition for Patient Clinics needing medicine optimisation. Clinical and Care Home Medication Reviews Identifying patient cohorts at high risk of harm from medicines. Review the use of medicines associated with unplanned hospital admissions. Management of common/minor/self-limiting ailments caseload. Provide patient clinics for queries/concerns about medicines. Management of medicines following discharge from hospital, intermediate care and into Care Homes Answer relevant queries from GPs, PCN staff, other healthcare teams. Signposting to the appropriate healthcare professionals Support in producing and implementing a repeat prescribing policy. Contribute pharmaceutical advice for the development/implementation of services Interpret, present medicines data to highlight issues and risks to support decision making. Audits of prescribing directed by the GPs, feedback the results and implement changes. Implement changes to medicines that result from MHRA alerts, product withdrawal and other guidance. Implementation of local/national guidelines and formulary recommendations Provide education/training to GP teams. Work with GP teams to ensure compliance with CQC standards. Support public health campaigns by providing specialist knowledge. Liaise with other stakeholders and CCG colleagues on prescribing matters to ensure consistent patient care. Maintain strong links with services across the PCN/neighbouring networks.
