Clinical Pharmacist – Harness North PCN Join to apply for the Clinical Pharmacist – Harness North PCN role at NHS. Job Summary The Harness North PCN Clinical Pharmacist will join a dynamic and innovative Clinical Pharmacy team that has been established for several years and includes trainee pharmacists and pharmacy technicians. This is an excellent opportunity to be part of a valued team within the Harness network. Key Responsibilities Provide professional support and clinical expertise to the PCN and wider teams regarding medication queries, prescribing issues, and systems for safer prescribing. Supervise and support the CP team. Oversee key PCN deliverables. Conduct face‑to‑face or remote long‑term condition reviews, Structured Medication Reviews and Care Home Reviews. Lead delivery of quality improvement, population health management, and clinical audit for the PCN in pharmacy‑led clinical domains such as Impact and Investment Fund (IIF), Medicines Management (MM) in the PCN DES, and CQC compliance. Support practice teams with day‑to‑day functions such as medicines reconciliation, repeat prescribing, and queries from patients and teams, utilising digital tools (e‑consults, Docman, internal tasks). Assist key stakeholders in streamlining the CP service. Work closely with Clinical Directors and other stakeholders to shape and deliver the strategy and vision for the PCN. Key Priorities for the Role Position the Clinical Pharmacist as a principle student in the delivery of high‑quality, patient‑centred multidisciplinary care. Use the Clinical Pharmacist skill set to allow General Practitioner appointments to be reserved for appropriate caseloads, reduce waiting times, increase access to healthcare, improve screening and diagnosis of chronic and common ailments, and reduce A&E attendances. Provide direct care to patients with long‑term conditions, minor ailments, and other clinical areas within scope of practice. Offer professional input to practice teams working to improve patient outcomes and reduce avoidable hospital admissions. Liaise with external pharmacists (community and hospital) to ensure accurate and effective medication management. Provide pharmaceutical support to practices so that medication‑related processes are reviewed, best‑practice principles applied, and evidence‑based values embedded. Assist patients in the safe, effective use of medicines to improve outcomes and quality of life. Ensure optimal benefit for prescribed medication through high levels of concordance achieved by support for patients and carers. Support the care of complex patients in their own homes or in nursing/residential accommodation as part of a multidisciplinary team. Use the Clinical Pharmacist skill set to ensure NHS resources are used cost‑effectively to maximise outcomes while reducing wastage. Provide professional leadership on matters relating to medicine optimisation and quality improvement. Qualifications Pharmacy Degree Registration with the General Pharmaceutical Council (GPhC) Completion of the Pharmacy Education Pathway (PCPEP) through the Centre for Pharmacy Postgraduate Education Registered non‑medical prescriber Desirable Qualifications Postgraduate diploma in Clinical Pharmacy or equivalent Experience At least 12 months of experience working in a PCN/practice Experience with clinical management systems, especially Emis & System One Knowledge, Understanding & Skills Relevant theoretical and practical knowledge of Primary Care Networks, General Practice and evidence‑based medicine An appreciation of NHS agenda, national policies and government targets and quality improvement indicators Understanding of the current PCN DES specifications Proficiency with Microsoft 365 software Awareness of business intelligence systems to support management of patients in primary care settings Ability to work under pressure and manage unpredictable issues on a daily basis Capability to plan, manage, monitor, advise and prescribe medication for patients in core areas, including long‑term conditions Ability to organise workload, multitask and meet tight deadlines Desirable Knowledge & Skills Awareness of PCN budget management and funding systems Relationships Work autonomously and within a team Support equality, diversity and rights of patients, carers and colleagues Train practice staff to ensure efficient prescribing and medicines management procedures Influence, motivate and persuade prescribers to comply with recommendations or agreed course of action where barriers exist Lead on quality improvement workstreams and deliver quality outcomes Support Foundation, Entry, Trainee pharmacists and Pharmacy Technicians with development and day‑to‑day practice Support Senior CP team and proactively seek innovative ways to improve prescribing practice and PCN development Flexible approach to working patterns and adaptability to changing priorities Self‑motivated and proactive Desirable Relationship Skills Provide education and training to PCN teams Communication & Information Excellent interpersonal and communication skills, both written and verbal Clear, concise record‑keeping and report‑writing skills Ability to communicate complex and sensitive information in an understandable form to a variety of audiences Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and will require a Disclosure to the Disclosure and Barring Service.
