Clinical Pharmacist Join to apply for the Clinical Pharmacist role at NHS. Summary The post holder will provide primary support to general practice staff with prescription and medicines management, including the repeat prescription system, acute prescription requests, and medicines reconciliation. The role involves delivering high‑quality pharmaceutical services across K&W sites, practices, care homes and house‑bound patients, through face‑to‑face, telephone, and online consultations. Key Responsibilities Clinical reviews and medication input, including targeted reviews for high‑risk patients, triage of minor ailments, and inpatient discharge reconciliations. Support practices with medicines optimisation, risk stratification and quality assurance audits, implementing drug withdrawals and NICE guidelines. Facilitate repeat prescribing processes, coordinate with community and hospital pharmacies, manage re‑authorisation, and reduce medication wastage. Teach and advise clinical workforce, provide newsletters, and support public health campaigns. Manage pharmaceutical information and liaise with CCG medicines management teams. Contribute to multidisciplinary reviews, clinical pathways, and continuous improvement initiatives. Maintain an open‑door policy for medication advice and urgent issues. Assist with education of callers and education of patients on safe medication use. Person Specification Essential Qualifications Master of Pharmacy (MPharm) degree and one‑year pre‑registration training, accredited by the General Pharmaceutical Council (GPhC). A‑level and GCSE qualifications with grades A‑C. Desirable Qualifications Ideally 2 years of experience in general practice. Salary and Contract Salary: £23‑£25 per hour. Contract type: Permanent.
