Clinical Coordinator

Role Overview We are seeking two dynamic and compassionate Clinical Coordinators to join our team in a newly created role supporting children and young people with Special Educational Needs and Disabilities (SEND) as part of our commitment to delivering on SEND improvement priorities. Responsibilities Coordinate clinical input for SEND pathways, ensuring timely and high‑quality clinical contributions to Education, Health and Care Plans (EHCPs) in line with statutory requirements. Facilitate multi‑agency collaboration as key liaison between health services, education and social care to support integrated planning and delivery for children and young people with SEND. Support SEND inspection improvement actions, delivering on specific improvement priorities identified through SEND inspections to ensure compliance and service transformation. Provide oversight and guidance, offering expert advice to frontline teams to manage complex cases and reduce variation in care. Monitor performance and quality, tracking service delivery against key performance indicators and contributing to audit, evaluation and continuous improvement. Engage with families and professionals, supporting effective communication with families, carers and professionals to ensure coordinated care and positive outcomes.
