We are looking for a proactive and dynamic individual, capable of working across various teams and services within the organisation and across statutory services. The individual needs to be an excellent communicator, who can prioritise, manage their own caseload and work autonomously.

Key skills required include:

  • Awareness and understanding of young adult palliative care
  • Ability to engage and interact with our teenagers and young adults at our group sessions
  • Supporting young adults and their families with transition support
  • Working across adult palliative care services and settings as part of our palliative transition work within adult hospices across the region
  • Awareness and understanding of adult safeguarding, consent and capacity
  • Ability to work as part of a small close-knit team
  • Appropriate clinical skills to support the teams service offer
  • Additional clinical support and oversight for wider community team.
  • Most importantly, a sense of humour, quick thinking, creative thinking and never taking yourself too seriously

Within the small young adult team, we cover transition support, palliative transition and we have our own caseload of teenagers and young adults who get their own bespoke offer tailored to their needs focusing on the benefits of social interaction.

This role is full-time (37.5hrs) and can be worked flexibly across 4 days, with an expectation to work flexibility over a 7-day period if needed, with an option to work at home 1 day a week; dependent on need.

The main responsibilities would centre around working across social group sessions, day to day key work for the case load, support with palliative transition, attending joint clinic appointments at adult hospices, general networking and collaborative working with various teams across the NHS. There will also be some collaborative working with the wider team of care coordination and planned community care to provide clinical support and caseload work including care planning.
