Practice Nurse (Band 6) – Cornwall Health for Homeless We are seeking a full‑time Practice Nurse to join our team of GPs. The role supports primary care for people experiencing homelessness in Cornwall, working in clinic sites and through outreach across the county. Provide health assessments, chronic disease management, vaccinations, wound care and sexual/reproductive health services. Deliver health education and screening for infectious diseases. Offer lifestyle and wellbeing advice and advocate for patients. Engage patients flexibly, building trust with those facing barriers to care. Coordinate care with multidisciplinary teams, outreach services and social care. Participate in GP‑led clinics in Truro, Camborne, Penzance and outreach clinics countywide. Flag and address health inequalities to improve overall health outcomes. Working hours are Monday to Friday, with some outreach work possible. Compassion and flexibility are key.
