Practice Nurse – Health for Homeless Band 6, full‑time (37.5 hrs/week) permanent Practice Nurse role at Phoenix MCT, Cornwall Health for Homeless, based in Truro. Job Overview Cornwall Health for Homeless is a GP practice that provides a range of services to people without homes. Clinics operate in Truro, Camborne and Penzance and are open to those who are street homeless or in temporary accommodation. The service works with St Petrocs, Coastline Homeless Service and other local and national statutory groups to support patients to live more stable lives and access suitable accommodation. As a Practice Nurse, you will support the GP team in delivering primary care tailored to the homeless population, including health assessments, chronic disease management, vaccinations, wound care and sexual/reproductive health services. You will also deliver health education, screen for infectious diseases and provide lifestyle and wellbeing advice, engaging patients flexibly and building trust with those who may face barriers to care. Main Duties Provide primary care tailored to people experiencing homelessness, including health assessments, chronic disease management, vaccinations, wound care and sexual/reproductive health services. Deliver health education, screen for infectious diseases, and support lifestyle and wellbeing advice. Engage patients flexibly, building trust with those who may face barriers to care, irregular attendance or trauma histories. Work closely with multidisciplinary teams, outreach services and social care to coordinate care, advocate for patients, and ensure continuity, aiming to reduce health inequalities and improve overall health outcomes. Participate in outreach clinics across the county as needed, supporting the team in extended or flexible working patterns. Qualifications & Experience RGN level 1 registration. Diploma level specialist training, experience, short courses or equivalent experience. Substantial post‑registration experience in acute, general practice or community teams. Experience working autonomously, yet as a team member through education and experience. Experience with a wide range of medical conditions. Evidence of autonomous practice. Desirable Criteria Prescribing qualification. Teaching qualification (e.g. City & Guilds, HEA Mentorship Module or similar). Experience in triage, minor illness/injury, community nursing or long‑term conditions. Skills & Knowledge Ability to provide specialist, evidence‑based nursing care to a wide range of patients. Leadership and management skills. Strong communication and team working abilities.
