The Community Nursing Service within North East Lincolnshire provides nursing care and support to patients, families and carers in a variety of community settings, supporting patients to remain in their own homes, preventing hospital admission, facilitating early hospital discharges, and managing end‑of‑life care. The Community Nursing Service works closely and in partnership with Primary Care Networks, Secondary Care, Social Care, Specialist Nurses, and other healthcare staff to provide a quality service personalised to meet individual needs. The service supports and assists patients to manage ill health and long‑term conditions including elements of disability, thus allowing them to have the best possible quality of life, while maintaining dignity and respect. Qualifications We are looking to appoint a qualified Registered Adult Nurse 1st Level with additional post‑registration nursing qualifications at Advanced Practice MSc level, including V300 Independent prescribing and Long Term Conditions, along with community experience. Responsibilities Carrying out physical examinations Deciding on and carrying out treatment, including prescribing of medicines, or referring patients to an appropriate specialist Using expert knowledge and clinical judgement to identify potential diagnosis and make a final diagnosis Using extensive practice experience to plan and provide skilled, competent care that meets patients' health and social care needs, involving the healthcare team as appropriate Ensuring the provision of continuity of care, including follow‑up visits Assessing and evaluating, with patients, the effectiveness of the treatment and care provided and making changes as needed Liaising with health professionals to offer additional help and expertise with patient consent Prescribing medicines, or liaising with GPs regarding medication management Benefits Access to further training and development Ongoing supervision and support Contribution to the future development of services Employee‑owned social enterprise culture valuing trust and engagement Commitment to above 95% customer satisfaction and national recognition Highest ISO accreditation and commitment to reducing carbon footprint Excellent terms and conditions including optional membership to a contributory pension scheme, Independent Provider closed access to the NHS pension scheme, lease cars, a full induction programme, and extensive CPD/development opportunities Recognition of continuous service from partner Health and Care organisations for the purposes of annual leave entitlement Supportive employer with family‑friendly and flexible working Equal opportunities employer, safeguarding the welfare of vulnerable adults, and a Disability Confident employer Work schedule: 24 hours a week, required to work a rota pattern including weekends/bank holidays between 08:00 and 16:30.
