The Community Nursing Service within North East Lincolnshire provides nursing care and support to patients, families and carers in a variety of community settings, supporting patients to remain in their own homes, preventing hospital admission, facilitating early hospital discharges, and managing end‑of‑life care. The Community Nursing Service works closely and in partnership with Primary Care Networks, Secondary Care, Social Care, Specialist Nurses, and other healthcare staff to provide a quality service personalised to meet individual needs. The service supports and assists patients to manage ill health and long‑term conditions including elements of disability, thus allowing them to have the best possible quality of life, while maintaining dignity and respect. Qualifications We are looking to appoint a qualified Registered Adult Nurse 1st Level with additional post‑registration nursing qualifications at Advanced Practice MSc level, including V300 Independent prescribing and Long Term Conditions, along with community experience.
