Job SummaryWe have a vacancy for a Clinical Pharmacist to support a number of services being delivered across our Frailty Service based at the Jean Bishop Integrated Care Centre in Hull.Role OverviewOur Clinical Pharmacists work with a team of doctors, nurses and therapists providing a comprehensive geriatric assessment to patients seen in:The Jean Bishop Integrated Care CentreCare homesOur community bedded unitsOur frailty hospital at home (virtual ward)We can offer part‑time hours (2 and 3 days) and flexible working to support our current services and projects within the Frailty team.Key Responsibilities & ExpectationsExperience of performing structured medication reviews is preferable but not essential. We provide training as part of an individualized induction programme and embed you within an experienced frailty team. The successful pharmacist will focus on patient‑centred care, have an interest in frailty, and demonstrate excellent communication and organisational skills.Applicants are welcome from all pharmacy sectors. You will receive professional support from our established Medicines Service team based in Hull.You must have an independent means of transport to travel around the geographic area our services operate across and to work flexibly to meet the needs of the organisation.
