We are currently building on our Placed Based Model and developing our key links with our local partners and voluntary agencies and we are looking for someone who would be keen to help us build on our progress. This role involves working with people from 18 onwards, who have needs as per the Care Act 2014, we also work with carers and informal carers. This role can include working with Children such as Young Carers, therefore an enhanced DBS will be required. Newark and Sherwood Social Work Team is built up of 3 teams, Newark North, Newark South and Sherwood. As a whole unit we work together to support each other where demand arises. In addition to supporting each other in our development, vision and wellbeing. This role will be Line Managed by the Sherwood Sub Team, but there will be expectations for the suitable candidate to undertake work in other areas in Newark and Sherwood where demand arises. There may also be opportunities where tasks may require you to undertake work wider across Nottinghamshire and outside of the County where relevant. Key Responsibilities Conduct Care Act assessments, support planning, and risk assessments Work creatively with adults to promote independence, wellbeing, and strengths-based practice Collaborate with external agencies in a hybrid working environment (part office, part home-based) Undertaking of Mental Capacity Assessment and Best Interest Decisions, Undertaking Work set by the Court of Protection Completing Risk Enablement Plans and Assessments Partaking in the Duty Rota System Completing of Enquiries under Section 42 of the Care Act Attending MDT's with organisations and partner agencies Essential Requirements Registered with Social Work England In-depth knowledge of the Care Act 2014 and the Mental Capacity Act 2005 Effective communication, negotiation, organisational, and IT skills Ability to work autonomously and manage a varied work load Commitment to interprofessional collaboration UK driving licence and access to a vehicle (taxi service available if required due to disability) What We Offer A supportive team environment with regular formal and informal supervision Ongoing training and development to help you reach your potential Equipment to allow you to work in a hybrid manner Regular Continued Professional Development Sessions Flexible working practices to support your wellbeing Contact For an informal chat or more information, please contact: Team Manager Jordan Power 0115 8041188 or email at Jordan.power@nottscc.gov.uk Disabled applicants who meet the essential shortlisting requirements will be guaranteed an interview.
