Join Our Team as Heart Failure Specialist Nurse Applications are invited to apply for an 8 Month Secondment for a Band 7 Highly Specialist nurse role in Heart Failure Nurse. This is an exciting opportunity to join an established and innovative heart failure team. We are constantly changing and evolving the service to provide the best possible evidence‑based care to the residents of Salford. Working in both the community and hospital setting—to provide continuity in patient care throughout their journey. Remote patient monitoring—preventing hospital admissions, providing reassurance for patients and support to the patient caseload. Interprofessional working with the renal team—as we are currently developing a new cardiorenal service. Weekly MDT with consultant Cardiologists. Monthly MDT with a variety of nurse specialists, ACPs and consultants in Renal, Elderly Care, Diabetes, Cardiology and Palliative care. Main duties of the job The role of the Highly Specialist Heart Failure Nurse is to provide specialist advice to support the assessment, planning and implementation of programmes of care, this is within the inpatient and outpatient/community setting, autonomously and within the specialist team. To carry out specialist nursing procedures. Accountable for own actions, not directly supervised. Lead specialist in a defined area of nursing care. To provide specialist education to other staff and learners. Undertakes research and leads clinical audits in own specialist area. To provide comprehensive inpatient assessment and planning of care to inpatients via the referral system to facilitate appropriate treatment and contribute to reduced length of stay, they will display compassion and empathy to the patients during their hospital stay. To work within the Heart Failure team and contribute to the multidisciplinary to optimise patient care. To work within the outpatient/community setting in the form of nurse led clinics to provide comprehensive assessment and treatment of patients with heart failure. Actively monitor and participate within our caseload of patients. The specialist nurse will actively participate in Virtual Ward, whereby patients' blood pressure and weight are monitored post discharge to prevent decompensation. Person Specification Qualification Registered Nurse with current NMC registration (Part 1 Adult / General Level 1 or 2) 1st Degree Evidence of Professional / clinical knowledge in Heart Failure Proactive involvement in Mentorship and development of nurses Effective communicator, able to discuss sensitive and complex information 2 years post registration and 3 years within specialty Willing to undertake Learning as required for the role Non Medical prescriber Achieved or working towards masters level qualification Training, Skill and Experience Significant post registration experience in coronary care setting Significant post registration experience in coronary care setting in a leadership role Experience of teaching clinical skills Experience of classroom/teams teaching/presenting Wide-ranging in‑depth knowledge of clinical issues in CHD, Atrial fibrillation/Flutter, syncope, palpitation, CIEDs Significant experience of working within cardiology Understanding of Clinical Governance and Health & Safety issues Experience of clinical audit Willingness to work flexible shifts to deliver the service Familiar with concept of Nurse led consent Good understanding of cardiac care development Skills Abilities Utilise highly developed physical skills, dexterity, hand/eye co‑ordination for clinical skills e.g., assembling clinical equipment, venepuncture, giving IV injections, cardiopulmonary resuscitation, and defibrillation Be able to undertake patient observations and monitoring and use relevant equipment safely Ability to travel to different geographical locations across the Trust footprint in line with service needs Full UK driving licence with willingness to use own car for work purposes Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
