Job Summary Join Our Team as Heart Failure Specialist Nurse Applications are invited to apply for an 8 Month Secondment for a Band 7 Highly Specialist nurse role in Heart Failure Nurse. This is an exciting opportunity to join an established and innovative heart failure team. We are constantly changing and evolving the service to provide the best possible evidence-based care to the residents of Salford. This includes: Working in both the community and hospital setting – to provide continuity in patient care throughout their journey. Remote patient monitoring – preventing hospital admissions, providing reassurance for patients and support to the patient caseload. Interprofessional working with the renal team – as we are currently developing a new cardiorenal service. Weekly MDT with consultant Cardiologists. Monthly MDT with a variety of nurse specialists, ACP's and consultants in Renal, Elderly Care, Diabetes, Cardiology and Palliative care. Main Duties of the Job The role of the Highly Specialist Heart Failure Nurse is to provide specialist advice to support the assessment, planning and implementation of programmes of care, this is within the inpatient and outpatient/community setting, autonomously and within the specialist team. To carry out specialist nursing procedures. Accountable for own actions, not directly supervised. Lead specialist in a defined area of nursing care. To provide specialist education to other staff and learners. Undertakes research and leads clinical audits in own specialist area. To provide comprehensive inpatient assessment and planning of care to inpatients via the referral system to facilitate appropriate treatment and contribute to reduced length of stay, they will display compassion and empathy to the patients during their hospital stay. To work within the Heart Failure team and contribute to the multidisciplinary team meeting to optimise patient care. To work within the outpatient/community setting in the form of nurse led clinics to provide comprehensive assessment and treatment of patients with heart failure. Actively monitor and participate within our caseload of patients. The specialist nurse will actively participate in Virtual Ward, whereby patients' blood pressure and weight are monitored post discharge to prevent decompensation.
