Dream Medical is working with a respected Practice in the Bristol area and urgently require full time Advanced Nurse Practitioner ASAP. An opportunity has arisen for an Advanced Nurse Practitioner to join a dedicated and friendly team. The role is to provide holistic health care for patients including those with undifferentiated and undiagnosed problems. Your main responsibilities are to triage and manage patients requesting home visits, assess then implement the appropriate programme of care including the option of face to face clinical consultations ad nurse led prescribing. You would assist in improving the management of long term conditions, including CHD, asthma, diabetes and COPD, as well as improving smoking rates, sexual health, minor illness, physical health and healthy eating. This role is full time, salary is negotiable depending on experience plus additional benefits. If you are looking for a permanent post then this could just be the perfect role for you.
