Old Swan Health Centre Group Practice is a friendly, forward-thinking GP practice based in Liverpool, providing care to over 9,200 patients. We are committed to delivering high-quality, patient-centred care to our local community. We are seeking an experienced Advanced Nurse Practitioner (ANP) to join our multidisciplinary team.The successful candidate will play a key role in providing expert clinical assessment, diagnosis and treatment for patients presenting with a wide range of conditions. As part of our total triage appointment system you will be responsible for reviewing incoming patient requests and determining the most appropriate clinical pathway to ensure timely and effective care.Main duties of the jobMain Duties and ResponsibilitiesParticipate fully in the total triage appointment system, assessing and prioritising patient contacts.Undertake autonomous consultations, including face-to-face and telephone appointments.Provide evidence-based clinical assessment, diagnosis, treatment and prescribing.Order and interpret investigation to support clinical decisions.Manage acute and long-term conditions within scope of general practice.Work collaboratively with GPs, nurses, pharmacists, physiotherapists and other team members to optimise patient outcomes.Contribute to clinical audits, quality improvement and service developments.About usOur team includes 5 partners, 3 salaried GPs, 4 Registrars,ANP, 2 Practice Nurses, HCA, Physiotherapist, Practice Manager, ReceptionManager, Secretary and a team of Receptionist/Care Navigators andAdministrative Staff.We pride ourselves as being one big work family, providing each other with the necessary support and guidance to achieve the aims and goals of the PracticeJob responsibilitiesThe post holder will:Provide the choice of direct access to an ANP, in the Practice, over the telephone as well as home visits for the practice population.Make professional autonomous decisions for which she/he is accountableProvide clinics within the Practice for patients presenting wit undifferentiated undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision making, to establish a diagnosis and management planInstigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reportsFully qualified independent prescriber to prescribe safe, effective and appropriate medication as defined by current legislative frameworkProvide sage, evidence based, cost effective, individualised patient careOffer a holistic service to patients and their families, developing where appropriate an on-going plan of care/support, with emphasis on prevention and self-careProvide health and well-being through the use of health promotion, health education, screening and therapeutic communication skillsRefer patients directly to other services/agencies as appropriateWork with relevant colleagues to ensure that National Service Framework (eg Coronary Heart Disease/Older Peoples/Diabetes/Mental Health) are being deliveredContribute to the practice achieving its quality targets to sustain the high standards of patient care and service deliveryPerson SpecificationExperienceRegistered Nurse with current NMC Registration.MSc in Advance Clinical Practice (or equivalent).Independent Non-Medical Prescriber (V300).Experience of working in a GP Practice or Primary CareDisclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. #J-18808-Ljbffr
