Our nursing team at Jubilee is growing and we are looking to recruit an experienced Advanced Nurse Practitioner to join our established team. Our ANPs work alongside GPs to provide remote triage of patients by telephone or eConsultations, they conduct telephone and face to face patient appointments including history taking, clinical assessment, diagnosis, treatment and referral where appropriate. There potentially could be some home visiting required with this role. To apply you should be an autonomous Practitioner within your current role. At Jubilee you will work as part of the practice multidisciplinary team, delivering care within your scope of practice to our patient population. You will be based in our multidisciplinary hub within the surgery alongside our pharmacy team, mental health, network and GP. You must have a full masters qualification to apply for the substantive post advertised but we will consider evidence of masters level study and equivalent experience in the role. You should be an autonomous clinical decision maker in your current role. About us Jubilee is a large North Liverpool Practice serving a population of 10,350 patients. Job responsibilities KEY RESPONSIBILITIES Clinical Care Act as Advanced Nurse Practitioner, demonstrating advanced clinical competence and a knowledge base beyond those associated with conventional nursing roles. Undertake consultations with patients as an autonomous practitioner and using own clinical judgement to diagnose, treat or refer patients. Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan. Make direct referrals to primary, secondary and social services within locally agreed pathways, guidance and protocols. Work with patients and their carers in order to support compliance with and adherence to prescribed treatments. Ensure care plans are in place, including emergency health care plans following discussion with the patient and family members. Ensure that patients receive high quality clinical care, delivered in a timely manner. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, within scope of practice. Ensure complete and accurate documentation of each and every patient contact. Work as an autonomous practitioner and as part of multi-disciplinary and multiagency teams in order to ensure patients needs are met. Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care Professional. Adhere to the NMC Code of Professional Conduct and at all times work within the scope of professional practice. Ensure that professional practice adheres to organisational and NMC policies, procedures and guidelines. Maintain a professional manner and act as a positive role model to other members of the Primary Care team. Maintain confidentiality with regard to information pertaining to patients and staff. Ensure that the service interfaces with all other departments in a professional and productive manner, providing an effective service to partner organisations and other service providers. Where required provide supervision and mentoring to other staff in order to help them develop their knowledge, skills and abilities. Deliver care according to NICE guidelines and evidence-based care. Initiate and participate in the maintenance of quality governance systems and processes across the organisation and its activitiesCommunication. Communicate effectively with patients and families regarding complex information about their care. Provide advice and information to patients, carers and their families where appropriate. Communicate effectively and work collaboratively with medical, nursing, allied health professionals and operational staff to ensure delivery of a co-ordinated service. Promote and demonstrate effective communication networks within the organisation, with partner organisations and other service providers. Clinical Governance and Service Improvement. Contribute to the development and implementation of clinical audits identifying areas of best practice and areas for improvement. Provide support and supervision to assist others to develop knowledge, skills, and abilities. Participate in quality improvement within the service. Contribute to the ongoing development of the service to address identified needs. Adhere to Company policies, procedures and guidelines. Educational/Personal Development. Contribute to the development and maintenance of an excellent learning environment. Contribute to the development of other members of the nursing team. Adhere to professional registration requirements. Participate in a self defining developmental plan to maintain and develop clinical skills and ensure your own professional development. Undertake and complete all statutory and mandatory training. Participate in an appraisal and regular performance reviews. Management of Risk. Manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients. Monitor work areas and practices to ensure they are safe and free from hazards and conform to health, safety and security legislation, policies, procedures and guidelines. Apply infection-control measures within the practice according to local and national guidelines. Advocate for policies that reduce environmental health risks, are culturally sensitive and increase access to health care for all. Equality and Diversity. Person Specification Qualifications NMC registration Have completed, or be in the process of completing a relevant RCN accredited advanced nursing practice at Masters (MCs) level Experience working in Primary Care Experience Strong Clinical Practice and skills Excellent verbal and written communication skills Work effectively as a team member Good organisational skills demonstrated by delivering own work with effective time management and prioritisation Demonstrate strong analytical and objective thinking skills Primary Care Experience Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. £47,810 to £68,631 a year Equivalent AFC Band 7-8b based on qualifications and experience #J-18808-Ljbffr
