Position Health Care Assistant – North Tees & Hartlepool Homefirst Response Service Overview We are inviting applications for a highly motivated and dynamic Health Care Assistant to join North Tees & Hartlepool Homefirst Response Service. This service will provide care to patients within a range of settings, including patients’ own homes and residential care homes. Key Responsibilities Provide a whole person approach to the care and management for patients in this setting. Work autonomously, with a high level of knowledge in community care, with specific interest in palliative care, long‑term conditions, catheterisation and wound care. Liaise with a variety of health and social care providers and other agencies to ensure the provision of high quality care management to patients and their carers. Qualifications & Experience High level of knowledge in the field of community care. Specific interest/experience in palliative care, long‑term conditions, catheterisation and wound care. Ability to work autonomously and provide whole person care.
