At Exmoor Surgery, patients always come first.Job DescriptionJob Responsibilities:Practice as an independent prescriber within the scope of practice.Conduct face-to-face and telephone consultations for patients with minor illnesses.Provide medication review services at the practice and during home visits to local nursing homes.Offer advice and support for patients with self-limiting conditions.Encourage cost-effective prescribing practices within the practice.Review all safety alerts and ensure the necessary actions are taken.Reconcile medicines following patients’ discharge from hospitals or clinic appointments.Review medications for newly registered patients.Implement and maintain a robust repeat prescribing system.Provide expertise on medication monitoring and embed the system.Conduct targeted medication reviews for high-risk patients.Reauthorize medications for repeat prescribing within the scope of practice.Identify patients suitable for repeat prescribing and liaise with relevant clinicians as needed.Implement safe prescribing systems for vulnerable patients to improve compliance.Support clinicians in managing patients with drug and alcohol dependencies.Signpost patients to the appropriate healthcare professional.Assist in meeting QOF targets.Administer influenza and travel vaccines.No Canvassers or AgenciesPerson SpecificationQualifications:A minimum of five GCSEs including maths, English language, and one science.Disclosure and Barring Service Check: This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975.
