Your role includes promoting the safe, effective, and cost-efficient use of anticoagulants to enhance patient care continuity across primary and secondary care interfaces. You will represent the Pharmacy department on various committees, including the Thrombosis Committee, the Immunoglobulin Assessment Panel, and the South London Cardiovascular Network. Additionally, you will model high standards of pharmaceutical service and support divisions in managing anticoagulant expenditures. This position also involves supervising and training junior pharmacists and other pharmacy staff, as well as providing operational and clinical support across the Trust.Key ResponsibilitiesClinical Services and Patient CareParticipate in the front-line provision of pharmacy services and act as a clinical pharmacist in relevant areas, joining consultant ward rounds as required.Serve as a clinical role model, ensuring safe, clinically effective, and cost-efficient use of medicines.Lead and develop specialist anticoagulation services, including pharmacy-led anticoagulation clinics.Coordinate and assess training programs for new pharmacists regarding anticoagulant use.Promote prudent use of anticoagulants in line with local and national guidelines.Provide comprehensive clinical pharmacy services to wards, including medicines reconciliation and the maintenance of dispensing systems.Participate in consultant post-take ward rounds to address pharmaceutical care needs and ensure follow-up plans are communicated effectively.Be accredited for discharge prescription transcribing to facilitate patient discharge.Support the investigation of medication-related incidents and contribute to action plans to address issues with anticoagulants.Develop and implement strategies to enhance safe medicines use across the Trust.Liaise with primary care colleagues to share patient medication issues.Leadership and Service DevelopmentLead the appropriate use of anticoagulants across the Trust, setting and monitoring quality standards.Represent the pharmacy in service developments within clinical haematology, ensuring alignment with Trust and departmental objectives.Report on output, performance, and activity to the Clinical Director of Pharmacy.Identify and secure resources to enhance pharmacy services.Develop and adhere to guidelines and policies for cost-effective and safe anticoagulant prescribing.Collaborate on the development of Integrated Care Pathways and Patient Group Directions for clinical haematology and anticoagulation services.Ensure effective provision of pharmacy services by liaising with senior pharmacy staff.Division SupportManage anticoagulant expenditure through monitoring, forecasting, and audits, providing feedback to service and general managers.Identify cost-saving opportunities for anticoagulants and implement systems to achieve these savings.Educate healthcare professionals on the cost implications of medicines, particularly anticoagulant therapies, and promote evidence-based prescribing.Contribute to the preparation of business cases for service developments in Pharmacy, Clinical Haematology, and Anticoagulation services.Actively participate in the Thrombosis Committee and division meetings.Staff Management and TrainingOversee the induction and development of pharmacy staff, conducting regular performance reviews and managing various HR-related matters.Support the education and training of healthcare professionals, contributing to clinical training programs.Continuously update management and professional knowledge through training programs, self-education, and practice research.Arrange departmental meetings to keep staff informed on current issues and development needs related to anticoagulation therapies.Serve as a postgraduate tutor for pharmacy staff as needed.Application ProcessInterested candidates should apply for this position by submitting their application through NHS Jobs.
