Band 7 Clinical Practitioner – Planned Care, Locality Hubs Passionate about transforming care for people living with frailty? An exciting opportunity has arisen within the Locality hubs for a Band 7 Clinical Practitioner for Planned care to support the Locality Integrated Neighbourhood team. The Integrated Neighbourhood Long Term Condition practitioner will act as a generalist clinical expert, providing advanced knowledge and care for patients living with long‑term conditions (LTCs), including frailty. The role requires high levels of clinical judgement, discretion and decision making in complex care situations. Working closely within Primary Care Networks (PCNs) and as part of the wider Integrated Neighbourhood Teams, the Integrated Neighbourhood Long Term Condition Practitioner will support proactive, person‑centred care to improve outcomes and enhance quality of life for individuals with long‑term conditions. The role will support the co‑ordination and delivery of services through the Integrated Neighbourhood Teams to help citizens stay healthy, manage their own conditions and remain well in their own homes. This includes initiating treatment plans, monitoring care and working in partnership with other health, social care and community services to promote independence, prevent deterioration and reduce unnecessary reliance on services. We support people with multiple, often complex, health and care needs who are receiving care from many parts of health and social care. The team is linked to numerous support organisations and services, allowing specialist help if needed. We work closely with GPs, hospital clinicians and a wide range of community and voluntary sector services. Through an established data sharing agreement, referrals can be made directly to our team, enabling us to co‑ordinate more joined‑up, person‑centred care. Responsibilities The successful candidate will play a key role in leading high‑quality, person‑centred care, working collaboratively across multidisciplinary teams, and contributing to service development and improvement. Responsibilities include: Initiating and monitoring patient treatment plans, and working with other services to promote independence. Co‑ordinating care for patients with long‑term conditions and frailty across primary, secondary and community services. Providing advanced clinical expertise and decision making in complex care situations. Acting as a generalist clinical expert within the Integrated Neighbourhood Teams. Participating in workforce development and improvement initiatives. Qualifications The successful candidate will have: Professional registration (NMC/HCPC). Qualified non‑medical prescriber. Completed a health assessment module. Strong leadership and communication skills. A commitment to person‑centred, evidence‑based practice. UK Home Office Visa Sponsorship Requirements In accordance with UK Home Office requirements, Band 2 clinical and non‑clinical roles and Band 3 Non‑Clinical roles are not eligible for sponsorship. All non‑clinical roles (Agenda for Change Bands 7‑9) that meet the prescribed skill level (RQF 6) and salary threshold will be eligible for sponsorship.
