Job SummaryTo be responsible for delivery of practice nursing services, working as part of the practice’s multidisciplinary team who provide care within their scope of practice to the entitled patient population. The practice nurse will be responsible for health promotion, chronic disease management, health prevention, well‑woman and well‑man clinics, and support the management team in reviewing clinical policy and procedure.Primary ResponsibilitiesDevelop, implement and embed health promotion and well‑being programmesImplement and evaluate individual treatment plans for chronic disease patientsIdentify, manage and support patients at risk of developing long‑term conditions, preventing adverse effects on patients’ healthProvide routine nursing care to patients as required in accordance with clinical‑based evidence, NICE and the NSFProvide wound care (ulcer/Doppler etc.) to patientsProvide travel medicine servicesRequest pathology services as necessary & process pathology resultsProvide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as requiredMaintain accurate clinical records in accordance with legislation and effective codingMaintain chronic disease registers & undertake LTC condition reviews (Red/Amber/Green Clinics) e.g. Asthma, COPD, Diabetes, LD, DementiaDevelop, implement and embed well‑woman and well‑man clinicsChaperone patients when requiredAssist GPs with minor surgery when requiredPrioritise health issues and intervene appropriatelySupport the team in dealing with clinical emergenciesRecognise, assess and refer patients presenting with mental‑health needsSecondary ResponsibilitiesAct as the audit lead, effectively utilising the audit cycleMonitor and ensure safe storage, rotation and disposal of medicamentsSupport junior members of the nursing team, providing guidance when necessaryParticipate in local initiatives to enhance service delivery and patient careSupport and participate in shared learning within the practiceContinually review clinical practices, responding to national policies and initiatives where appropriateParticipate in the review of significant and near‑miss events applying a structured approach (e.g. root cause analysis)Person SpecificationExperience (Essential): Experience of working in a primary care environment and chronic disease management.Experience (Desirable): Experience as a practice nurse or community nurse.Qualifications (Essential): Registered Nurse.Qualifications (Desirable): Postgraduate diploma or degree in Primary Care.Employment DetailsContract: Permanent – Full‑time.Pay scheme: Salary depends on experience.Location: 56–60 Loampit Hill, London, SE13 7SX.Reference number: U0101‑25‑0015.EEO StatementAll staff at TLCP have a duty to conform to equality, diversity, and inclusion.
