We have an opportunity for an enthusiastic practicenurse to work with a cohesive primary care team serving 23,000 patients in StJohns Wood, Primrose Hill and Maida Vale.We offer comprehensive NHS primary care services andwork in a purpose-built state of the art premises in a lovely part of London,within the Hospital of St John and St Elizabeth.Main duties of the jobTobe responsible for the implementation of processes for the effective managementof patients with long-term conditions using evidence-based practice includingcare for elderly and housebound patients.Workingas a senior member of the practice multidisciplinary team, the post holder willensure nursing services are delivered effectively to the entitled patientpopulation.ThePractice Nurse will be responsible for a number of clinical areas such asinfection prevention and control, health promotion, chronic disease management,health promotion, well women and well man clinics, as well as activelysupporting the practice management team in the reviewing and delivery ofclinical policy and procedure.About usWe are a well-established General Practice dedicated to providing high-quality healthcare services with a patient-focused approach. There may be, onoccasion, a requirement to carry out other tasks. This will be dependent uponfactors such as workload and staffing levels:To act as or support the Infection Prevention Control (IPC) leadand to work to the IPC Policy providing or supporting the implementation ofaudits, training, support and evidence of compliance to the practice managerTo produce or support the production of the IPC annual statementin conjunction with the practice managerTo develop, implement and embed an effective care managementprogramme for the frail and housebound patients registered at the practiceTo assess the needs of patients ensuring the provision ofhealthcare is appropriate, incorporating evidence-based practiceTo develop, implement and embed health promotion and wellbeingprogrammesTo implement and evaluate individual treatment plans for chronicdisease patients that promote health and wellbeingTo identify, manage and support patients at risk of developinglong-term conditions, preventing adverse effects to patients healthAs required, to provide routine nursing care to patients inaccordance with clinical based evidence, NICE and the NSFAs required, to diagnose and manage acute and chronic conditionsTo prescribe medication for patients in accordance with PGDs andown scope of practiceTo review patient medications to enhance complianceTo provide guidance to patients in the use of prescribed andover-the-counter medications regarding side effects and dosagesTo co-ordinate the provision of travel medicine servicesTo request pathology services and process pathology results asrequiredTo oversee the provision of chronic disease clinics, deliveringpatient care as necessary, referring patients to secondary/specialist care asrequiredTo maintain accurate clinical records in conjunction with extantlegislationTo ensure read codes are used effectively by all members of thenursing teamTo ensure the appropriate maintenance and use of chronic diseaseregistersTo develop, implement, embed and participate in well woman clinicsTo develop, implement, embed and participate in well man clinicsTo ensure compliance with the practice chaperone policyTo assist GPs with minor surgery when requiredTo prioritise health issues and intervene appropriatelyTo support the team in dealing with clinical emergenciesTo recognise, assess and refer patients presenting with mentalhealth needsTo implement vaccination programmes for adults and childrenTo support patients in the use of their prescribed medicines orover-the-counter medicines (within own scope of practice)To liaise with external services/agencies to ensure the patient issupported appropriately (vulnerable patients etc.)To delegate clinical responsibilities appropriately (ensuring safepractice and the task is within the scope of practice of the individual)To support the clinical team with all safeguarding matters, inaccordance with local and national policiesTo understand practice and local policies for substance abuse andaddictive behaviour, referring patients appropriatelyTo deliver opportunistic health promotion where appropriateInaddition to the primary responsibilities, the Practice Nurse may be requestedto:Develop and review audit protocols and process for the practice,effectively utilising the audit cycleDevelop, review and implement nursing protocols in conjunctionwith the partnersSupport all members of the nursing team, providing guidance whennecessary, conducting appraisals and training needs analyses as requiredParticipate in local initiatives to enhance service delivery andpatient careSupport and participate in shared learning within the practice inorder to improve patient careContinually review clinical practices, responding to nationalpolicies and initiatives where appropriateParticipate in the review of significant and near-miss eventsapplying a structured approach, i.e., root cause analysis (RCA)Coordinate the nursing team rota, ensuring sufficient staff areavailable to meet patient needsLead the planning and implementation of changes within the nursingteam, enhancing the provision of services to patientsSupport the practice team with the reviewing and implementation ofpractice policies and protocols, ensuring conformance to extant legislationPerson SpecificationQualificationsEducated to GCSE O level or equivalent.Recognised qualification as a registered NurseTrained in the use of Clinical systems used in General PracticeA demonstrable commitment to professional developmentExperience of Primary CareDisclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. #J-18808-Ljbffr
