Location: Marfleet Group Practice, Clifton House Medical Centre, The Avenues Medical Centre and Campus Health CentreAcclaim Primary Care Network (PCN) is a GP led organisation established in 2023. It has four member practices, listed above, with a combined list size in excess of 38,000 patients. The PCN provide enhanced services to patients across all four GP Practices, for example, weekly Care Home ward rounds, same day access clinics and Home Visits for acutely ill patients.Please note we do not hold a Sponsorship Licence.----------------------------------------------------------------------------Interview Date: Thursday 8th January 2026.----------------------------------------------------------------------------Main duties of the jobWe are looking for a proactive, self-motivated person to help progress and build on this relatively new role, as a GP Assistant. With your knowledge, skills and experience, its your chance to make your mark in primary care locally. You would support the PCN with the facilitation of clinical, holistic, and proactive care for our patients, whilst also undertaking admin tasks to support the GPs and other members of the primary healthcare team. The preferred candidate will understand working in primary care, with previous experience of working in a healthcare setting and strong administrative skills. Experience in the use of SystmOne would be an advantage.On a day-to-day basis, you will work collaboratively with colleagues across the PCN, supporting the Care Home Team and the Practice teams to deliver high quality healthcare. You will promote health and wellbeing to all patients; support in providing care for individuals with dementia, learning disabilities and long-term conditions, signposting, where appropriate, to relevant wider service across the locality. More specifically your duties will include, Phlebotomy, taking past medical history, observations (BP), ECGs, Learning Disability reviews, prediabetic reviews, and work as part of the team delivering vaccinations. Your duties will complement and support the workforce across the PCN. Your duties will complement and support the workforce across the PCN.Core ResponsibilitiesAs part of the role, you will be expected and trained to help:Admin work, such as but not limited to the following:Extracting all information from clinical letters that need codingDirectly dealing with all routine clinical postArranging appointments, referrals and follow up appointments of patientsClinical workGeneral Clinical Duties including but not limited to.Preparing patients, taking a brief history and basic readings.Dipping urine, phlebotomy and taking blood pressure and ECGsCarrying out annual reviews, pre-diabetic reviewsExplaining treatment procedures to patients, including arranging follow up appointmentsHelping the GP liaise with outside agencies, for example, arranging for on-call doctors to provide advice on the phone, or to arrange admission, allowing the GP to continue with their consultation(s)Forming a part of the Care Home team, undertaking review work ups for the residents of the PCNs aligned Care Homes.Person SpecificationQualities and AttributesAn understanding, acceptance, and adherence to the need for strict confidentialityAble to work on own initiative.Able to work under pressure and remain calmFlexible and adaptableProactivityOtherFlexibility of Working HoursDrive and have access to transport to attend visits or sites across the PCN, this may include home visitsRequired to have a Disclosure Barring Service (DBS) checkExperienceExperience within a healthcare setting and/or similar roleExperience in caring for othersExperience in GP PracticeExperience of the use of SystmOne clinical systemKnowledge and skillsAbility to provide quality patient careGood interpersonal and communication skillsGood organisational skillsGood team working skillsKnowledge of medical terminologyAbility to work as a team member and autonomouslyEffective time management (Planning & Organising)Basic phlebotomy.Ability to undertake ECGs, BPs, and basic observations.Ability to undertake Learning Difficulty annual reviews for patients.Ability to carry out pre-diabetic appointments for newly diagnosed patients.Qualifications2 GCSE grade A-C English and MathsPrevious experience of general clinical duties.NVQ Level 2 in Health and Social Care, and willing to work towards Level 3, or equivalentDisclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. #J-18808-Ljbffr
