Nurse Case Manager

This role is ideal for nurses with a background in ICU, HDU, or complex care who are ready to embrace more autonomy, flexibility, and a genuine work–life balance — all while continuing to make a real impact in people’s lives.
This is a full time, permanent role working as a Nurse Case Manager (complex care), in the Merseyside, Cheshire, and Lancashire region. The role is to work Monday to Friday, 9am – 5pm, with no nights, weekends, or bank holiday working. The salary is £43,000 per annum. You must be a driver with car for this community role.
What You’ll Do
As a Community Registered Nurse, you’ll play a key role in supporting and overseeing high-quality care for individuals with complex health needs, including those with:
Spinal or neurological injuries
Acquired brain injuries or trauma
Epilepsy, learning disabilities, or genetic disordersYour responsibilities will include:
Assessing, reviewing, and maintaining bespoke care packages
Providing hands-on clinical training and supervision to care staff
Offering expert guidance and support to clients and their families
Collaborating with health and social care partners (ICBs, hospitals, case managers, and other professionals)
Participating in a paid on-call rota on a rotational basisEvery day brings something different — and the satisfaction of knowing you’re making a lasting difference in your community.
About You
To apply, you will be;
A Registered Nurse (Adult or Paediatric) with current NMC registration
Experienced in ICU, HDU, or complex care environments
A confident communicator and natural leader
Empathetic, self-motivated, and passionate about delivering person-centred care
