Opportunities are available in:East Intensive Case Management in Psychosis (ICMP) Community Mental Health Team – 35 hours per weekWest Intensive Case Management in Psychosis (ICMP) Community Mental Health Team – 35 hours per weekOlder Adults (OAMH) Community Mental Health Team – 35 hours per weekEarly Intervention in Psychosis (EIP) Community Mental Health Team – 17.5 hours per weekOxleas House Mental Health Unit (Queen Elizabeth Hospital) & Home Treatment Team – 35 hours per weekOur TeamsIntensive Case Management in Psychosis (ICMP East and West Teams)ICMP provides a flexible service to all Greenwich residents between the ages of 18 and 65 years, within the psychosis clusters who currently require secondary community mental health treatment and support.The service focus is on social inclusion, recovery, working towards self-management, and offering individuals choice and involvement regarding their treatment, whilst actively encouraging their participation in the planning and implementation of their care. This results in a strength based personalised wellbeing plan which supports the individual’s referral back to primary care.Team Locations:ICMP East:Ferryview Health Centre, 27 John Wilson Street, Woolwich, SE18 6PZ.ICMP West:68 The Heights, Charlton, SE7 8JH.Early Intervention in Psychosis (EIP)EIP provides assessment, treatment and support to people living within the borough of Greenwich who are experiencing psychosis for the first time. Support is provided in the community for up to three years. Discharge destinations include primary care (GP), a different type or longer-term secondary care mental health team (ICMP or Community Mental Health Rehabilitation and Enablement Service) or on occasion another EIP team (if the service user relocates). A core value of the service is its strong ethos of hope and optimism and whole-team commitment to enabling recovery through the provision of individually tailored, evidence-based interventions for both service users and their support networks.Given that the team often work with people experiencing contact with mental health services for the first time, this requires a considered and skilled approach in terms of engagement.The successful candidate will be joining a highly skilled, motivated, and performing team, evidenced by annual EIP audit result data.Team Location:Ferryview Health Centre, 27 John Wilson Street, Woolwich, SE18 6PZ.Older Adults Community Mental Health TeamThe Greenwich Older Adults Mental Health team is a specialist integrated multidisciplinary team for adults over the age of 65 experiencing severe and enduring mental health difficulties or have a dementia diagnosis. The team provides health and social care assessments, support and advice for carers and a variety of interventions to maximise independent living, and to promote mental wellbeing.The team providesupportthrough care coordination and a programme of short-term psychological groups directed to older people assessed for the service. All professionals work collaboratively with the inpatient unit, crisis, and assessment team, to assess the support that the individual needs in the community, to contribute to the multi-agency panel to consider those who may need a package of care and to discuss safe discharge from hospital.Team Location:Memorial Hospital, Shooters Hill, Woolwich, SE18 3RG.What we look for:All social workers will be expected to be able to manage a caseload and to work co-productively with service users their families and carers to ensure that a strength based, person centred approach remains at the core of all interventions.To be part of a multidisciplinary team that consists of social workers, occupational therapists, psychiatrists, psychologists, support workers, lived experienced practitioners, community psychiatric nurses and administrative support. To work autonomously with individuals, carers, and families to deliver a range of assessment and care interventions that promote mental health wellbeing and strengths based personalised outcomes for people, whilst promoting independence. You will be expected to undertake statutory functions of the social work role (Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983 (2007), Children Act 1989 (2004) and other related legislation), responding to requests for those who present with care andsupportneeds:Senior Social Work/AMHP Role and Responsibilities (this list is not exhaustive)Use their social care knowledge and skills to guide intervention with service users, families, and carers.Undertake care act assessments, support planning and reviews under the Care Act 2014.Undertake the AMHP role on behalf of the local authority and participate in the Greenwich Central AMHP Team borough wide AMHP rota (pro rata).Undertake and provide consultation and support on Safeguarding Adults and Children procedures and to act as a Safeguarding Adults Manager, Enquiry Officer, MARAC Representative and Think Family Champion (where appropriate).Undertake work with individuals and their families, to develop and maintain self-help, improve problem solving and coping capabilities towards improved health and wellbeing.Participate in Section 117 discharge and care planning processes and meetings.Promote and undertake interagency collaboration and networking with the local authority, housing, third sector organisations, voluntary agencies, and community networks in order to mobilise resources.Complete statutory documentation and reports within the required time scales.Assess the needs of groups who are difficult to engage or those from excluded groups (i.e. nil recourse, drugs/alcohol).Undertake mandatory and relevant training to maintain their social care practice, knowledge, and skills.Your job satisfaction is important to us - tosupportyou in your AMHP role you will receive:1 to 1 reflective professional supervisionand AMHP post assessment supervisionAccess to a manager who is AMHP qualifiedAn AMHP mentor to provide additional support in your first year of employment18 hours (3 days) of AMHP approval training in each yearA structured induction to Greenwich AMHP serviceUp to 2 days in each 5 year period to complete your AMHP re-approval portfolioOpportunity to be involved in the practice education of social workers and AMHPs undertaking professional trainingAccess to Social Work, AMHP and Reflective Practice Forums for peer group discussion reflection and supervisionA welcome meeting with the Head of Social Care and Mental Health Lead Social Worker to begin to discuss and plan for your continued professional development pathwayAccess to protected time to undertake the requirements for your Social Work England registrationAccess to the Royal Borough of Greenwich learning portal, training programmes, reflective workshops, and eventsStrong partnership working between Royal Borough of Greenwich Health and Adults Services, Integrated Commissioning Board and Oxleas NHS Foundation Trust (Health Service Journal - Trust of the Year 2023 and The Sunday Times – Best Places to work 2023).
