The Senior Social Worker will assess and intervene in complex cases, supporting individuals and communities while promoting independence, choice, and safety. Key Responsibilities Provide professional social work services to adults, their families, and carers in line with statutory requirements Engage with individuals, families, and communities to assess needs and implement interventions that promote choice, control, and independence Manage complex situations, balancing support with risk management, collaborating with senior colleagues and professionals Develop support plans using a strengths-based approach, considering family, friends, and community resources Lead discussions regarding Personal Budgets and Direct Payments, supporting informed decision-making and ownership of plans Proven experience as a social worker, ideally in community response or adult social care Sound knowledge of the Care Act 2014 and relevant statutory guidance Strong assessment, risk management, and intervention skills Excellent communication, collaboration, and leadership abilities Why Hammersmith?
