Practice Nurse – GP Summary An exciting opportunity has arisen for an experienced Practice Nurse to join a progressive and supportive GP practice providing high-quality, patient-centred primary care. We serve a medium-sized patient population and consistently achieve excellent clinical outcomes and Quality and Outcomes Framework (QOF) results. Main Responsibilities Working as part of an established multidisciplinary clinical team, you will provide comprehensive nursing care and manage patients with long-term conditions. Responsibilities include: Chronic disease management (Diabetes, Respiratory, Cardiovascular conditions) Health promotion, screening, and lifestyle advice Immunisations and vaccinations across all age groups Cervical cytology and women’s health Travel health and sexual health advice Complex wound care and treatment room duties Contributing to clinical audits and service improvement initiatives Supporting junior colleagues and sharing best practice Accurate record keeping using a clinical IT system (TPP SystmOne or equivalent) Active involvement in Primary Care Network initiatives About the Practice We are a well-established training practice with a diverse and experienced clinical team including GPs, Advanced Practitioners, Pharmacists, Practice Nurses and Health Care Assistants. The team is supported by efficient administrative staff and benefits from modern, purpose-built premises. The practice promotes a collaborative, learning-oriented culture and encourages professional development, with opportunities to develop specialist interests within primary care. Person Specification Essential Qualifications & Experience Registered Nurse (NMC) Proven experience in General Practice Nursing Competence in managing chronic diseases (e.g. Diabetes, Respiratory, Cardiovascular) Trained in cervical cytology Experienced in immunisations and vaccinations Skilled in complex wound care Familiar with electronic clinical systems Desirable Mentor or teaching qualification Additional clinical interests or specialist skills relevant to primary care Other Requirements Commitment to ongoing professional development Enhanced DBS check (required prior to appointment) Employment Details Full-time or part-time hours available Competitive salary based on experience #J-18808-Ljbffr
