About the Role An exciting opportunity has arisen to join the expanding Integrated Transfer of Care Hub as a Discharge Nurse. You will work as part of a wider team whose energy and focus is to facilitate safe and timely discharges from The Mid Yorkshire Teaching NHS Trust. We are seeking an enthusiastic, responsible, and professional individual who would join a highly motivated, experienced and dynamic team. The successful candidate would be able to organise their workload, be flexible in their approach to changing needs and demands of the organisation, follow instructions under pressure and use their own initiative. Applicants need to have experience in a health care setting; ideally community however, acute care nurses would be considered. You will be working collaboratively with the multidisciplinary teams on the ward and in close partnership with system partners. This role will add value to the patient discharge experience and discharge process. There will also be an education side to this role around discharge planning for the ward teams. Participate in the development and delivery of a philosophy of care that supports and respects peoples diversity and rights. Relate to staff and patients in a confident manner, demonstrating good listening skills and the ability to work effectively as part of a team. Create, maintain and enhance effective working relationships with all nursing colleagues, the multidisciplinary team, health care professionals in primary and secondary settings plus local authority colleagues. Recognise areas f interpersonal conflict and develop a proactive stance in resolving issues. Develop an understanding of Risk Management in relation to discharge planning Maintain an awareness of national and regional initiatives Provide and participate in clinical supervision Strive for continuous improvements in delivery of high quality patient focused care. Participate in the implementation and maintenance of the Trust's Nursing Charter standards Demonstrate diplomacy and the ability to deal with colleagues and patients/carers in such a way as to avoid conflict Work alongside the MDT and in partnership with patients and their carers/families to utilize resources appropriately, offering guidance, support and education according to the patients individual needs. Refer patients to other health colleagues and other professional agencies as appropriate. Work alongside nursing colleagues and MDT to help to co-ordinate discharge planning as appropriate, to ensure a safe and timely discharge from hospital To work closely with an allocated number of wards to focus on discharge planning. Recognise and acknowledge own limits of knowledge and competence. Maintain up to date knowledge of clinical issues and practices Qualifications Essential person specification requirements Registered Nurse with extensive relevant clinical experience in hospital or acute setting. Experience Aware of current developments in discharge agenda Previous involvement in preparing reports for social services.
