This role offers an environment where professional growth is encouraged and supported.Job Description Summary (Main Duties and Responsibilities)Clinical Ward-Based Services: Deliver a high-quality, patient-focused clinical pharmacy service on wards, ensuring safe and effective medication use.Supporting Timely and Safe Discharges:Facilitate early discharge and reduce hospital length of stay by optimising drug therapy.Provide tailored education on medication use, support self-medication, and assess need for adherence aids.Offer comprehensive counselling to patients about medications prior to leave and discharge.Ensure continuity of care through robust discharge planning and communication with community healthcare providers.General Pharmacy Duties: Participate in wider clinical pharmacy and departmental responsibilities as needed, supporting the overall objectives of the Trust’s pharmacy service.We may close the advert early, so please apply as soon as possible.About UsLeicestershire Partnership NHS Trust (LPT) provides a range of community health, mental health and learning disability services for people of all ages, delivered through over 100 settings from inpatient wards to community.
