Go back Norfolk Community Health and Care NHS TrustCommunity Link NurseThe closing date is 14 December 2025An amazing and rare opportunity has arrived for a part time Band 6 Community Link Nurse in the NN4 Community Nursing teamWe are looking for an experienced registered community nurse to join our dedicated and highly professional team of Community/District nurses working in the Brundall, Acle, Hoveton and Wroxham areas covering the service seven days a week.This role will offer the opportunity for both clinical and leadership development, whilst working in a caring, friendly and supportive team.Main duties of the jobYou will be responsible for the assessment and treatment of complex patients with a wide variety of health needs of acute or chronic presentation, visiting them within their own homes, care homes, the travelling community and in clinics. We are looking for a nurse who is committed to delivering high standards of care. You will supervise Band 5 Community Nurses and other members of the nursing team, and work closely with the therapy staff to assist the patient to achieve maximal function and independence. In addition, you will support the Clinical Lead Nurse with managerial responsibilities.You will need effective communication skills and must be prepared to work flexibly and collaboratively to meet the needs of the service. It is essential to have evidence of professional competencies and have the ability to apply your knowledge and skills to clinical practice.
