Our West Lancashire Community Mental Health Team (CMHT) is seeking a Band 6 professional to join us on a full‑time basis. Overview This is your opportunity to manage a diverse caseload, providing essential assessment, planning, and review for service users with complex mental health needs. As a vital member of our multidisciplinary team, you will champion person‑centred care pathways and make a tangible impact on individual recovery and well‑being within the community. We are looking for an experienced professional with strong skills in risk assessment and management, and a passion for collaborative working. A full driving licence is essential. Main Duties Hold responsibility for a defined caseload, providing expert assessment and crafting personalised care plans for adults with complex mental health conditions. Deliver evidence‑based interventions, regularly review progress, and coordinate effectively with other health professionals to ensure seamless support. Conduct thorough risk assessments and develop robust management plans to safeguard service users. Provide community‑based care, travelling to meet service users in their homes and local settings. Qualifications & Experience Relevant professional qualification (e.g. RMN, Social Worker, OT) with current UK registration (NMC, SWE, HCPC). Significant post‑qualification clinical experience working with service users experiencing a range of mental health problems. Experience assessing, planning, implementing, and reviewing care in a community setting. Demonstrable knowledge and experience in clinical risk assessment and management. Ability to manage a complex caseload autonomously with effective prioritisation. Excellent verbal and written communication skills, with the ability to build therapeutic relationships. Full valid UK driving licence and access to a vehicle for community visits. Experience working within a Community Mental Health Team (CMHT). Work cohesively as part of a multidisciplinary team, contributing to case discussions and clinical reviews. Communicate effectively with GPs and other health and social care agencies to ensure integrated care. Key Working Relationships Service users, families, and carers. CMHT multidisciplinary team (Psychiatrists, Nurses, Psychologists, OT, Support Workers). GPs and primary care services. Community and voluntary sector organisations. Inpatient mental health services.
